At One Doorway, we have a close connection to hospitals, clinical teams, and discharge professionals to support NDIS participants to transition back into the community and settle into their new home.

Our Hospital Discharge Program assists individuals with a disability to come out of hospital into Medium Term Accommodation while a long-term solution can be arranged.
Depending on your circumstances, we can also work with you on a permanent housing option instead of medium term accommodation.

We assist individuals with a disability who are clinically ready for discharge from hospital to transition into new or temporary accommodation funded by the NDIS while providing them with the supports appropriate for their needs.

How does the Hospital Discharge Program work?

Our experienced client engagement managers can:

  • Collaborate with your clinical team and manage the process of discharge and your transition into your new home and community.
  • Collaborate with SDA providers to help you find your forever home. Source suitable Medium Term Accommodation and recruit a team of trained support workers to provide an individualised 24-hour support program with funding from the NDIS.
  • Develop person-centred support plans that meet the needs of each individual.
  • Ensure individual goals are captured and monitored within a person’s support plan.
  • Partner with other professions and allied health teams to assist participants with their NDIS plans and funding requirements.
